Geriatric Social Services Coordinator (GSSC)
Ethos is looking for a Geriatric Social Services Coordinator (GSSC)/Care Manager who will: conduct follow-up personal interviews to reassess client needs; coordinate services with providers; act as an advocate for clients in obtaining government and other services; and provide short-term crisis management support to clients. The GSSC attends primary care team meetings with the senior care organizations (SCOs) and meets documentation standards of the SCOs while performing responsibilities as part of a Home Care team.
LTSS Coordinator/PCA Skills Trainer
This is a split position intended to be equally divided between LTSS Coordinator and PCA Skills Trainer duties.
LTSS (Long Term Support Services) Coordinators are responsible for initial assessment and ongoing support of One Care members. One Care is a health care option for Massachusetts adults aged 21-64 who are eligible for both MassHealth and Medicare. The target population includes non-elderly adults with physical disabilities, developmental disabilities, serious mental illness and substance abuse disorders.
PCA (Personal Care Attendant) Program Skills Trainers provide Functional Skills Training to consumers of the MassHealth PCA Program. This program helps people with permanent/chronic disabilities keep their independence, stay in the community, and manage their own personal care by providing funds to hire personal care attendants (PCAs).
PCA Skills Trainer
The Personal Care Attendant Program is a MassHealth program that helps people with permanent or chronic disabilities keep their independence, stay in the community, and manage their own personal care by providing funds to hire personal care attendants (PCAs).
PCA Program Skills Trainers provide intake and orientation services to prospective consumers and/or their surrogates; maintain a caseload of consumers who are enrolled in the PCA Program and provide ongoing Functional Skills Training, by telephone and in-person, to ensure that the consumer and/or their surrogate are managing the program effectively.
