Please complete and return your membership agreement form as soon as possible. Once received, a welcome letter and membership card will be mailed to you. You can download and print a copy of the form HERE.
If you have not done so already, we ask that you complete the Member Intake Form included with the enrollment form, so that we may be able to better serve you and the members of JP@Home. You may also complete the Intake Form by click HERE.
